Student Application	Registration fees must accompany the application when turned in. This is a yearly form.
Recommendation Form (Pastor)	Must have pastor fill out and turn in. Possible exceptions for no pastor. First time students only.
Recommendation Form (Friend)	Must have two (2) friends fill out and turn in to the school. First time students only.
Previous School Information Request	First time students only.
Consent to Treatment	Must accompany Application. This is a yearly form.
Book and Computer Use Agreement	Must accompany Application. This is a yearly form.
Financial Plan	Must accompany Application. This is a yearly form.
Medical Exam Report	Required for grades one, four, seven, nine and all new students.
